Experteer Overview In this role you will support MAT access for individuals with opioid use disorder by engaging with EDs, jails, and outpatient sites. You’ll coordinate referrals, education, and action plans, ensuring patients connect to the highest level of care. You will work within a cross-functional team to address barriers to follow-up and maintain timely documentation. This position sits within the SOR 4 grant program, contributing to a mission of improving recovery outcomes in a fast-paced environment. Compensation / Benefits • Develop outreach and education lists with the team • Provide education and outreach to external entities in collaboration with TCD • Round in ED/jail and other settings to identify MAT-eligible patients • Maintain data on referrals and outreach activities • On-call MAT linkage during business hours if not present in ED/jail • Describe OUD MAT concepts to external entities • Obtain consent and complete MAT intake forms • Assess readiness (Stages of Change) and screen for co-occurring disorders • Verify insurance status and connect uninsured patients to financial counseling • Identify MAT resources and assist with follow-up appointments • Address barriers to follow-up (transportation, contact info, shelter) and coordinate social services • Inform ED/jail providers about MAT follow-up plans and reminders prior to appointments • Be present for in-person meetings during working hours • Other duties as assigned Tasks • Registered or Certified Counselor • Nonjudgmental, energetic, positive approach to SUD • Understanding of SUD as a disease and role of MAT • Exceptional oral and written communication skills • Proficient in Microsoft Office (Word, Excel, MS Project) • Ability to meet deadlines and manage multiple projects in fast-paced environment • Strong attention to detail and audit-trail documentation • Ability to build rapport with clients, vendors, and colleagues • Compliance with professional standards • Initiative and problem-solving in unforeseen situations • Satisfactory references, clean background check, and drug screen Key requirements • 401K match • Medical, Dental, Vision Insurance • Accident Injury, Hospital Indemnity and Critical Illness Plans • Company paid Short & Long Term Disability • Company paid Basic Life Insurance • Paid Time Off
