We are currently seeking an organized and devoted Service Coordinator to join our team!

JOB SPECIFICATIONS

PRIMARY RESPONSIBILITIES

  • Manage a range of services to residents, tenants, clients, and their families
  • Ensure a welcoming admission to the building by providing emotional support, information regarding day-to-day activities, and encouraging socialization for the resident. Assist in creating and maintaining an environment that achieves high-quality care (physical, mental, emotional, social and spiritual) of residents and maintain their independence and dignity to the highest degree possible. Establish and maintain effective communication systems with all customers and respond promptly to resident and family issues.
  • Participate as a member of the IDT (interdisciplinary team) to ensure adequate and appropriate flow of services. Promote the tenant’s well-being and satisfaction by supporting person-centered care in all areas of the resident’s life. Coordinate ongoing assessments to manage changes in clients’ physical and emotional needs.
  • Provide service coordination for residents to include arranging and/or referring appropriate support services. Plan interventions based on a variety of needs and requests. Implement and follow established resident services policies, procedures and practices. Use discretion and good judgement in recommending possible courses of action and decision-making.
  • Establish and maintain effective working relationships within Sholom and with outside agencies and organizations and participate with the various community constituencies to achieve mutually beneficial results
  • Coordinate resident services collaboratively with other professionals inside and outside Sholom.
  • Maintain a current, up-to-date knowledge of community resources.
  • Develop and maintain relationships with other agencies and community resources to ensure ease when directing referrals.
  • Serve as a resource for residents and families, providing ongoing support and advocacy.
  • Maintain accurate Social Services records and documentation.
  • Initiate arrangements for a discussion with resident and families regarding Advance Directives and Providers Order for Life-Sustaining Treatment (POLST).
  • Interact with internal departments pertaining to care and financial issues.
  • Carry out site responsibilities including but not limited to: working with fall prevention program; parking lot procedures; meal program logistics; surplus food program assistance.
  • Preform other duties as assigned.
  • Create and maintain property's monthly resident calendar and newsletter
  • Develop and facilitate educational and preventive health and wellness programs for residents, including ongoing programs and minimum of 3 original programs per month)
  • Attend Monthly RSC Staff meetings and Provided Educational training

Knowledge, Skills, and Abilities

  • Display high levels of customer service, responding promptly and thoroughly to the inquiries and needs of individuals both internal and external to the organization.
  • Demonstrate familiarity with available community resources. Able to establish cooperative referral relationships with other agencies.
  • Demonstrate excellent interpersonal skills to effectively build relationships, exhibit empathy, handle interactions at all levels, and respond appropriately in sensitive situations.
  • Possess the capability to work with highly emotional and stressful situations.
  • Understand and maintain the confidential nature of organization and client information, including and in accordance to HIPAA regulations.
  • Proven ability to effectively set and maintain boundaries when working with clients.
  • Possess effective problem-solving skills.
  • Demonstrate strong computer skills including Microsoft applications such as Word, Excel, and Outlook.
  • Possess a knowledge of state and federal laws related to mental health and government programs.
  • Able to display high levels of customer service, responding promptly and thoroughly to the inquiries and needs of individuals both internal and external to the organization.
  • Able to plan, prioritize, coordinate, and manage own work. Able to work unsupervised, make decisions, and solve routine problems independently, effectively, and creatively.
  • Possess the capacity to work as part of a team of interdisciplinary professionals, including Nurses, Home Health Aides, Therapy, Therapeutic Recreation, Nutrition Services, and other personnel.
  • Understanding when to effectively contact co-workers or others for answers to questions outside personal expertise.
