Responsibilities
- Deliver individual and family case management sessions using Skills Training/Development activities.
- Implement crisis intervention strategies to manage acute mental health emergencies effectively and ensure patient safety.
- Maintain accurate medical documentation and behavioral health records in compliance with HIPAA regulations; utilize medical terminology proficiently for clear communication.
- Collaborate with multidisciplinary teams on case management plans and coordinate care for patients in outpatient or inpatient settings.
Skills
- Proficiency in motivational interviewing techniques to foster patient engagement and motivation for change.
- Strong background in child & family case management, pediatrics, geriatrics, and working with people with disabilities.
- Skilled in crisis management, behavior management strategies, and crisis intervention protocols.
- Ability to work effectively within medical records systems while maintaining strict confidentiality under HIPAA guidelines.
