Position Summary:
The Discharge Planner is responsible for ensuring effective communication between services within the hospital and the community for the purposes of patient discharge. The Discharge Planner is responsible for discharge flow for all patients from inpatient level of care to safely be discharged and transitioned to their community and/or next level of care timely.
- Assists the Social Services Department with discharge planning including aftercare arrangements. Discharge planning includes locating and referring for services, scheduling follow-up appointments, and transportation arrangements from the hospital to next level of care.
- Assists with completion of patient satisfaction surveys, and clinical outcomes surveys (CABA/BASIS).
- Initiates follow up calls to external referral sources, including management of care point alerts.
- Assists with tracking discharge data: Compiles discharge reports and data. Audits and confirms that all discharge paperwork is accurate and complete prior to accompanying the patient to their safe transition home or to a community-based program.
- Ensure that discharge transition records are sent to receiving providers

Requirements
- Hgh School Diploman or GED, required
- Bachelors degreee, preferred
- One plus years of experience in a healthcare setting, behavioral health a plus
- Strong customer service skills
- Proficient in Excel
- Strong communiation skills
- Ability to multitask
- Stong organzational management skills
