Responsibilities:

• Provide social work services to residents, including crisis intervention, counseling, and case management

• Develop and implement individualized care plans in collaboration with the interdisciplinary team

• Conduct assessments and evaluations to identify resident needs and develop strategies for addressing those needs

• Collaborate with other departments, such as nursing, dietary, and activities, to promote resident well-being

• Coordinate services with community agencies and external providers to support resident needs

• Supervise and mentor social work staff to ensure excellent care delivery

• Participate in quality improvement initiatives and quality assurance activities

• Ensure compliance with regulatory requirements and internal policies

Requirements:

• Minimum 0 years of experience as a social worker in a healthcare setting

• Master's degree in Social Work (MSW) or related field

• New Jersey Social Work licensure (LSW, LCSW, or other applicable licenses)

• Excellent communication, interpersonal, and organizational skills

• Ability to work effectively in a fast-paced environment and prioritize tasks efficiently

• Strong problem-solving and decision-making skills

• Ability to maintain confidentiality and handle sensitive information
