Client Care Coordinator – Sacramento Region

As a Client Care Coordinator, you will be the primary guide for our clients’ services—helping them set meaningful goals, tracking their progress, supporting their caregivers, and ensuring their daily lives reflect dignity, choice, and person-centered planning. You will work closely with our SLS/ILS teams across Fairfield, Davis, and Sacramento to ensure each client receives exceptional support.
CLASSIFICATION:
Full-Time – 40 hours/week
Compensation dependent on experience.
*Regular travel between Fairfield, Davis, and Sacramento is required. A valid driver’s license, reliable vehicle, and insurance are mandatory.
Primary Duties & Responsibilities
- Manage a caseload of up to 20 clients (caseloads may be adjusted with COO approval).
- Conduct client orientations, 30-day reviews, quarterly/bi-annual/annual assessments, and ongoing service plan monitoring.
- Review and complete daily/weekly notes, timesheets, mileage logs, goal tracking, and other required documentation.
- Serve as the first responder for client-related field emergencies during scheduled work hours; after-hours situations require COO/CEO approval.
- Participate in the development and updates of Individual Program Plans (IPPs) and coordinate quarterly, semi-annual, and annual meetings.
- Provide hands-on training and coaching to clients and caregivers based on program domains, behavioral plans, and individualized goals.
- Oversee the selection, onboarding, and supervision of assigned direct support staff; ensure strong staff-to-client matches.
- Guide staff through planning, coordinating, documenting, and evaluating client progress toward program objectives.
- Evaluate program effectiveness, identify barriers to client success, and develop strategies to resolve challenges.
- Conduct home visits, maintain organized client files, and meet all internal Quality Assurance standards.
Qualifications
Education:
- Bachelor’s Degree in Social Services, Psychology, Human Development, or related field (preferred)
OR
- At least 2 years of experience working with individuals with developmental disabilities.
Experience:
- Minimum 2 years of direct support experience with individuals with developmental disabilities, medical needs, or psychiatric diagnoses.
- At least 1 year coordinating services in developmental services, social services, or a similar human-services system.
- Supervisory experience (1 year) strongly preferred.
Additional Requirements:
- Must pass a LIVESCAN background check before employment.
- Valid CA driver’s license, reliable registered vehicle, and auto insurance.
- CPR/First Aid certification (or willingness to obtain before working independently).
- Strong written communication skills; ability to write clear, detailed, and professional reports.
- Proficiency with Microsoft Office and comfort with digital documentation.
