Connected Health Care is currently seeking a compassionate and experienced Full-Time Hospital Social Worker for a prominent healthcare provider in Mammoth Lakes, CA. As a Social Worker within the hospital, you will provide vital social work services to patients and their families while working closely with a multidisciplinary team to promote holistic patient care. - Assess patient social, emotional, and psychological needs through individual and family interactions.
- Provide counseling and support services to patients and families facing medical challenges.
- Develop discharge plans in collaboration with healthcare providers to ensure smooth transitions.
- Offer resources and referrals to community services, including housing, financial aid, and mental health support.
- Assist with crisis intervention, helping patients and families cope with urgent or complex healthcare situations.
- Advocate for patients' rights and assist with navigating the healthcare system.
- Document all interactions and interventions in the medical record per hospital policies.
- Participate in care planning meetings and case reviews with medical staff.
- Ensure compliance with all applicable laws, regulations, and hospital policies.
- Master’s degree in Social Work (MSW) from an accredited institution preferred, Bachelors degree required.
- LCSW (Licensed Clinical Social Worker) required.
- Valid California state licensure as a Clinical Social Worker.
- Minimum of two (2) years of clinical experience in a hospital or healthcare setting preferred.
- Basic Life Support (BLS) certification preferred.
- Experience in medical social work, hospital-based settings, or healthcare environments.
- Strong knowledge of community resources and services in the Mammoth Lakes area.
- Experience with crisis intervention and working in a multidisciplinary team.
- Ability to work independently and manage multiple cases simultaneously.
