Connected Health Care is currently seeking a compassionate and experienced Full-Time Hospital Social Worker for a prominent healthcare provider in Mammoth Lakes, CA. As a Social Worker within the hospital, you will provide vital social work services to patients and their families while working closely with a multidisciplinary team to promote holistic patient care. Assess patient social, emotional, and psychological needs through individual and family interactions.
Provide counseling and support services to patients and families facing medical challenges.
Develop discharge plans in collaboration with healthcare providers to ensure smooth transitions.
Offer resources and referrals to community services, including housing, financial aid, and mental health support.
Assist with crisis intervention, helping patients and families cope with urgent or complex healthcare situations.
Advocate for patients' rights and assist with navigating the healthcare system.
Document all interactions and interventions in the medical record per hospital policies.
Participate in care planning meetings and case reviews with medical staff.
Ensure compliance with all applicable laws, regulations, and hospital policies.

Education and Certification Requirements:

Master’s degree in Social Work (MSW) from an accredited institution preferred, Bachelors degree required.
LCSW (Licensed Clinical Social Worker) required.
Valid California state licensure as a Clinical Social Worker.
Minimum of two (2) years of clinical experience in a hospital or healthcare setting preferred.
Basic Life Support (BLS) certification preferred.

Preferred Qualifications:

Experience in medical social work, hospital-based settings, or healthcare environments.
Strong knowledge of community resources and services in the Mammoth Lakes area.
Experience with crisis intervention and working in a multidisciplinary team.
Ability to work independently and manage multiple cases simultaneously.
