Job Overview
We are seeking an energetic and dedicated Program Director with a strong background in social work and nursing to lead our multidisciplinary healthcare teams. Responsibilities
- Lead and oversee the planning, implementation, and evaluation of healthcare programs involving social work and nursing services.
- Coordinate multidisciplinary teams including social workers, RN nurses, therapists, and support staff to deliver integrated patient-centered care.
- Develop policies and procedures aligned with healthcare standards such as NCQA standards, HIPAA compliance, and best practices in acute care, long-term care, hospice, and behavioral health.
- Monitor clinical operations ensuring adherence to safety protocols like aseptic technique, infection control, and sterile processing.
- Facilitate staff training on specialized areas including trauma medicine, dementia care, pain management, ICD coding (ICD-9 & ICD-10), EMR/EHR systems like Epic or Cerner, and medical documentation standards.
- Manage case reviews, discharge planning, and patient assessments to optimize health outcomes across inpatient, outpatient, home care, assisted living, and long-term care settings.
- Collaborate with community partners to enhance services for individuals with developmental disabilities or those requiring senior or memory care.
- Ensure compliance with legal regulations such as Workers’ Compensation Law while maintaining high-quality standards in hospice & palliative medicine and infectious disease care.

Requirements
- Valid RN license with current registration; experience in critical care environments such as ICU or Level I Trauma Centers preferred.
- Proven leadership experience managing healthcare programs involving social workers and nursing staff.
- Extensive knowledge of medical terminology, anatomy, physiology, diagnostic evaluation techniques including sonography or laboratory experience.
- Familiarity with EMR/EHR systems such as Epic or Cerner; proficiency in ICD coding (ICD-9 & ICD-10), CPT coding, and medical records management.
- Strong background in diverse clinical areas including emergency medicine, cardiology (cardiac catheterization), nephrology (dialysis), pediatrics (neonatal ventilator), geriatrics (memory/dementia care), behavioral health, pain management, and post-acute care.
- Experience working with vulnerable populations such as children with developmental disabilities or elderly patients requiring hospice or long-term care services.
- Excellent communication skills for effective triage assessments, patient education, team coordination, and community outreach.
- Ability to manage multiple priorities within fast-paced environments like hospitals (including trauma centers) or outpatient clinics while maintaining compliance with HIPAA regulations.
