Social Worker

Our Nursing Home Facility is seeking a caring and passionate full-time Social Worker to join our Social Services Team.

Responsibilites:

• Identifying individual social and emotional needs
• Developing and Maintaining individualized care plans
• Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
• Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident

Full Time: ( Monday-Friday 9am-5:30pm)

Requirements:

• Previous experience working in long-term care is preferred
• Bachelors degree in Social Work, Sociology, Psychology, or Human Services is REQUIRED.
