Job Description:

Presbyterian is currently hiring a Peer Recovery Support Specialist in Santa Fe, NM. This role focuses on providing Recovery Coaching to individuals facing mental health and substance use challenges.

As a Peer Recovery Support Specialist, you’ll help patients identify personal and environmental barriers to recovery, connect them with the recovery community, and serve as a mentor in managing both personal and family recovery. While not acting as a sponsor, you’ll model resilience, coping strategies, and a commitment to ongoing recovery.

Some key responsibilities:

  • Coordinates and communicates with hospital staff, funders and community partners as required.

  • Works independently and with a multidisciplinary team to provide services and deliverables as required by PHS and/or funders.

  • Engages patient toward recovery using Motivational Interviewing Skills.

  • Provides patient assessment(s).

  • Provides Naloxone education in partnership with nursing and medical personnel.

  • Identifies and provides community resources.

  • Works with ancillary/community providers to ensure seamless transition for continuity of care.

  • Supports patient in linkage with community appointment prior to discharge.

  • Regularly contacts patient after discharge.

Additional Job Description:

  • Education: High School Diploma/GED required

  • Knowledge of community resources required.

  • Register for Peer Certification class within 6 months after hire

  • Current NM drivers license is preferred

  • Certified Narcan educator preferred

  • Trained in Motivational Interviewing Skills preferred

  • Previous experience working in hospital or ED setting preferred

  • Experience working with confidential medical records and documentation preferred.

  • Effective written and verbal communication skills

  • Computer skills, familiarity with Microsoft Word, Excel, and Power Point

  • Familiar with readiness assessment tools and SBIRT.
