This role is crucial in ensuring that every referral is handled with accuracy, timeliness, and professionalism. As the Central Intake Coordinator, you will play a key role in guiding patients through the admissions process, communicating with referral partners, and supporting our internal teams—all while helping people feel welcomed and cared for from their very first interaction with Rivergrove.

Responsibilities:
Receive and enter SNF referrals from hospitals, providers, and community partners
Validate required demographic, insurance, and clinical information
Review for clinical and financial viability
Track referral status and ensure timely progression through the intake process
Communicate professionally with internal teams and referral sources
Maintain accurate, audit-ready documentation in admissions systems
Qualifications:
Strong organizational skills with excellent attention to detail.
Exceptional verbal and written communication skills.
Professional, courteous, and personable demeanor.
Ability to manage multiple tasks and prioritize effectively in a fast-paced environment.
Experience in healthcare, admissions, or patient coordination preferred.
Proficiency with computer systems and electronic admissions or healthcare software.
