Job Summary:This position is for a Home Health Licensed Masters Social Worker (LMSW) in the East Portland, OR and surrounding areas. The LMSW will provide social work services to patients in their homes, requiring independent transportation and the ability to manage a caseload with productivity standards. The role involves travel within a radius that may extend up to 60 miles, and adherence to specific requirements including COVID-19 vaccination and experience with Oasis charting.Location:Portland, Oregon, United StatesResponsibilities:- Provide social work services to patients in their homes.
- Manage a caseload and meet productivity standards (30 productivity points standard).
- Travel to patient homes within a service area, potentially up to a 60-mile radius.
- Document patient care and related duties appropriately.
- Coordinate with the patient care team.
- Maintain and clean equipment.
- Perform data entry into electronic medical database systems.
- Answer unit phone and directs calls accordingly.
- Perform selected patient registration activities.
- Coordinate equipment/supply needs. Notifies appropriate department of equipment malfunction.Required Skills & Certifications:- Active LMSW license in both Oregon (OR) and Washington (WA).
- BCLS certification.
- Minimum of 1 year of skilled home health experience.
- Oasis experience.
- Primary series COVID vaccination in addition to one booster.
- Driver's license, car insurance, and automobile for field workPreferred Skills & Certifications:- None specified.Special Considerations:- Traveler is expected to have their own transportation.
- Facility can call off UP TO one shift per pay period (every two weeks)
- The facility cannot accommodate 4x10s or 3x12s schedules.
- First-time travelers are accepted.
- Locals are considered within a 50-75 mile radius and will be accepted at the same rate.
- Guaranteed Hours: Facility can call off UP TO one shift per pay period (every two weeks)
- Traveler is expected to have their own transportation.
- System does a good job grouping clients minimizing drive and windshield time. Occasional maximum radius of roughly 60 miles (60-90 minute total drive time) per day.
- Facility requires primary series COVID vaccination in addition to one booster
- Oasis experience is requiredScheduling:
- 8-hour day shifts.
- No weekend requirement.
- No on-call requirement.
- Block schedules are not accommodated.
