Position Summary:

Overview:

We are seeking a compassionate and dedicated Licensed Master of Social Work (LMSW) to join our team at [Hospital Name]. This position is ideal for a skilled professional passionate about providing high-quality psychosocial support to patients and families in a hospital setting. The LMSW will be responsible for conducting assessments, offering counseling services, and helping individuals navigate emotional and social challenges related to health conditions and hospital care.

Key Responsibilities:

• Provide psychosocial assessments for patients to identify emotional, social, and mental health needs.
• Develop and implement treatment plans tailored to each patient’s unique needs.
• Provide individual, family, and group counseling sessions as needed.
• Work closely with interdisciplinary medical teams to coordinate patient care and ensure comprehensive support.
• Offer crisis intervention and support in high-stress situations.
• Educate patients and families about available community resources and assist with referrals as needed.
• Maintain detailed and accurate patient records in compliance with hospital policies and state regulations.
• Ensure all services are provided in alignment with ethical guidelines and professional standards.
• Participate in case conferences, staff meetings, and ongoing professional development activities.

Qualifications:

• Valid Licensed Master of Social Work (LMSW) license issued by the New York State Education Department (NYSED). Primary Source Verification required.
• Master’s degree in social work from an accredited institution
