Experteer Overview In this role you provide ambulatory pharmaceutical care under supervision, collaborating with health professionals to manage drug therapy, educate patients, and oversee medication preparation. You promote evidence-based, cost-effective care across clinical areas, aligning with the department’s goals. You lead medication reviews, adjust therapies under protocols, and monitor outcomes to optimize patient safety and effectiveness. You work with cross-functional teams to improve care quality and support ongoing professional education and performance improvement. Compensation / Benefits • Promote rational, cost-effective drug therapy in inpatient and outpatient settings in collaboration with physicians • Direct Pharmacist Residents, Interns, Technicians, and other staff • Take patient medication histories and develop treatment plans • Identify and resolve medication-related problems (indications, dosing, interactions, ADRs) • Initiate, adjust, monitor, and recommend drug therapy changes under protocol • Coordinate and interpret laboratory monitoring (drug levels, electrolytes) • Monitor and optimize patient outcomes, adherence, and drug utilization • Educate patients about disease state, therapy, and compliance • Provide education to providers and staff; participate in utilization reviews • Perform other duties as required Tasks • Clinical pharmacy residency or equivalent clinical patient care experience • Pharm.D or B.S. in Pharmacy • Pharmacist License (California) • Basic Life Support (AHA) • Knowledge of pharmacology, disease states, and medical terminology • Knowledge of aseptic technique and sterile IV/chemotherapy preparation • Knowledge of emergency medications and drug monitoring • Understanding of drug utilization review and regulatory requirements • Strong verbal and written communication; ability to use pharmacy computer systems and input labels • NPI number within one month of position start • Ability to work in a Labor/Management Partnership environment • ACLS certification may be required in some areas Key requirements •
