Job Title/Position: Hospice Social Worker

Glory Hospice has over 40 years of clinician experience in pain and symptom management. We are currently looking for a Social Worker to join our growing team in San Diego with an emphasis on North County.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

Responsibilities:

  • Collaborate with the hospice interdisciplinary team, which includes physicians, nurses, and other healthcare professionals, to develop hospice care plans
  • Conduct assessments of hospice patients' social and emotional needs and develop appropriate interventions
  • Provide individual and group counseling to hospice patients and families on issues related to hospice care, chronic illness, and end-of-life care
  • Connect hospice patients and their families with community resources and services
  • Case-manage hospice patients who require ongoing support and assistance
  • Document all interactions in the hospice patient's medical record according to departmental standards
  • Participate in hospice team meetings and quality improvement initiatives as needed

Qualifications:

  • Master's degree in Social Work from an accredited institution
  • 2+ years of experience working in hospice, home health, or with chronically ill patients preferred
  • State licensure as a Clinical Social Worker or eligibility to obtain licensure required
  • Knowledge of hospice care, medical terminology, and medical social work theory and practice
  • Strong communication and interpersonal skills
  • Ability to work collaboratively as part of a hospice interdisciplinary team
  • Demonstrated commitment to diversity, equity, and inclusion in the workplace

Job Type: Contract

Pay: $59,648.59 - $71,834.87 per year
