Job Title: Health Coordinator

Contract Duration: 4 Months
Schedule: Monday–Friday | 8:00 AM – 5:00 PM
Job Summary:
The Health Coordinator supports Medicare Care Management by conducting outbound and inbound telephonic outreach to complete Health Risk Assessments (HRAs), identify care coordination needs, educate members on benefits, and connect them with appropriate care management and community resources. The role ensures accurate documentation and compliance with Medicare regulatory requirements.
Key Responsibilities:
- Conduct telephonic HRA assessments and process mailed/faxed HRAs
- Educate members on plan benefits, gaps in care, and annual provider visits
- Assist with appointment scheduling and care coordination
- Route referrals to appropriate care or case management teams
- Escalate member concerns as needed and maintain regulatory documentation
Requirements:
- Strong communication, listening, and customer service skills
- Experience in managed care, population health, or care coordination
- Knowledge of Medicare regulations preferred
- Proficiency in Outlook, Excel, Word, and data entry
- Ability to multitask, work in a team environment, and provide empathetic member support
