Job Title: Medical Social Worker (MSW/LCSW) – Case Management
Location: Mission Veijo, California
Job Type: [13 Weeks Contract]
Shift: Day 5x8-Hour (08:00 - 16:30)
Overview:
We are seeking a dedicated Medical Social Worker (MSW/LCSW) to provide case management and care coordination for patients in various clinical settings. The ideal candidate will have experience in discharge planning, utilization review, and navigating community resources. This role requires strong assessment, documentation, and patient/family education skills.
Key Responsibilities:
- Conduct biopsychosocial assessments and develop individualized care plans
- Coordinate patient care across hospital units, home care, and community resources
- Facilitate discharge planning and contingency planning
- Assist patients with accessing benefits (Medicaid, Medicare, TANF, Food Stamps, Housing, Veterans programs)
- Provide crisis intervention and support for high-risk patients
- Document patient care, education, and progress according to plan of care
- Collaborate with multidisciplinary teams to ensure quality, cost-effective care
Qualifications:
- MSW required; LCSW preferred
- California state license required
- BLS certification required
- Case management or utilization review experience preferred
- Knowledge of community resources and specialized patient populations (e.g., CHF, COPD, dementia, psychiatric disorders, HIV/AIDS)
Preferred Skills:
- Experience in acute hospital settings (ICU, NICU, L&D, Medical-Surgical, Pediatrics)
- Familiarity with EPIC documentation system
- Understanding of legal and ethical standards (NASW Code of Ethics, Advance Directives, ADA)
