CARE COORDINATOR

BI-LINGUAL (ENGLISH/SPANISH)

Role Description

This is a full-time, on-site role for a Care Coordinator/ Bi-lingual (English/ Spanish), located in Memphis, TN. The Care Coordinator generally meets with the client(s) and the client's family (foster, adoptive or biological) at least once monthly, with one face-to-face visit every 90 days. At the time, telehealth visits are permitted to replace the face-to-face contact. The Care Coordinator's hours of work will vary depending on the individual needs of his/her caseload but are generally able to be scheduled in a manner that does not exceed 40 hours in each week. The Care Coordinator will be responsible for managing and coordinating client treatment plans, assessing client needs, maintaining clear and effective communication with clients and families, and collaborating with healthcare providers to ensure high-quality care. Additional responsibilities include providing excellent customer service to address client concerns and improve client satisfaction.

  • Strong Communication skills to effectively interact with clients, families, and healthcare teams.
  • Ability to be a team member and thought leader.
  • Proficiency in Customer Service to respond to client needs and ensure satisfaction.
  • Organizational and problem-solving skills to manage complex cases.
  • Ability to work collaboratively in a healthcare setting.
  • Bachelor's degree in a behavioral health related field is preferred.
  • Experience in behavioral health or similar healthcare settings is an advantage.
