Job Description:

This position is based out of our Chico, Cohasset Rd. Women's Health Center location. This role will primarily serve patients with perinatal mood disorders.

The Behavioral Health Therapist provides culturally competent clinical services to a diverse caseload consisting of children, adults, couples, and families. This clinical role within an integrated healthcare setting, conducts screenings and assessments; implements evidence-based interventions; offers psycho-education; engages in treatment planning, evaluates level of patient risk, and submits referrals to ancillary services as needed. The Behavioral Health Therapist works collaboratively with the multidisciplinary team to support patient treatment and care needs.

RESONSIBILITIES INCLUDE:

  • Conduct initial assessment and administer required standardized screening tools.
  • Gather relevant information necessary to accurately diagnose and treat patient; such as, mental status exam; functional impairments; behavioral health, substance use and trauma history; patient strengths and potential barriers to accessing services.
  • Join with patients to develop individualized treatment plans aimed at addressing identified mental health symptoms of concern and presenting problems.
  • Employ culturally competent, trauma-informed, best practices during psychotherapy.
  • Make essential referrals and connections to community resources through care coordination and by helping patients prioritize needs.
  • Support in warm handoff and provide crisis intervention/emotional stabilization for patients when need is identified by other department providers.
  • Document patient encounters within Electronic Health Record in accordance with compliance requirements and NVIH procedures (laws, regulations, ethical standards).
  • Participate constructively in Continuous Quality Improvement (CQI) activities within the Department, such as peer review, staff meetings and case conferencing.
  • Maintain caseload of patients; coordinates with receptionist staff regarding daily schedule; and meets/exceeds department productivity standard.
  • Establish trust and rapport to develop and sustain professional relationships with patients, providers, and partners.
  • Other duties as assigned.

Experience and Skills:


QUALIFICATIONS
:

  • Must hold current license or registration with the Board of Behavioral Sciences (LCSW, LPCC, LMFT, ACSW, APCC, AMFT)
  • Experience with crisis counseling and working with adults, children and families in an outpatient setting desired
  • Has demonstrated the ability to work with diverse populations (experience working with Native Americans is desirable)
  • Two (2) years therapy experience desired.
  • Demonstrates excellent interpersonal skills and ability to work as part of a multidisciplinary team.
  • Demonstrates understanding of age-related or specialty issues and developmental needs for each population served by adapting style, treatment and plan to each population.
  • Excellent written and oral communication skills, strong organizational problem-solving and analytical skills.
  • Ability to understand and adhere to established policies, procedures, protocols, and the ability to make timely and sound decisions.
  • Work independently under limited supervision, prioritizes work assignments.
  • Reliable Transportation and ability to adhere to regular attendance.
