```Duties```
As a Medical Social Worker, you will be responsible for providing support and assistance to patients and their families in a healthcare setting. Your duties will include:

- Conducting patient assessments to determine their social, emotional, and psychological needs completes the Comprehensive Assessment within five days of the patient’s election of hospice care
- Developing and implementing care plans to address the identified needs of patients
- Providing counseling and emotional support to patients and their families
- Assisting patients in accessing community resources and support services

-Assists with advance directives, funeral planning, discharge planning, family counseling and may assist in changes in the level of care as indicated by the patient or family’s needs.
- Collaborating with healthcare professionals to coordinate patient care
- Advocating for patients' rights and ensuring their needs are met
- Providing crisis intervention and management as needed

```Skills```
To excel in this role, you should possess the following skills:

- Strong knowledge and experience in senior care, patient care, hospice, and dementia care
- Proficiency in conducting patient assessments and developing care plans
- Familiarity with kinnser emr systems used in healthcare settings
- Applied behavior analysis skills to assist patients with behavioral challenges
- Experience working with geriatric patients is a plus
- Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare professionals
- Ability to handle crisis situations calmly and effectively

-Meets mandatory continuing education requirements of the Hospice and licensing board

-Attends IDG/IDT meetings, participate in the patient care planning process and coordinating and collaborating with the IDG/IDT to promote coordination of patient care
