JOB DESCRIPTION
This is a merit hire position. The hourly rate for this position is $20.00 per hour. This position is covered under a collective bargaining agreement. Dauphin County Area Agency on Aging is currently accepting applications for the full-time, day shift position of Aging Care Manager 1. This position may develop care plans for older adult consumers through an evaluation of the consumer’s social contacts and supports, their activities of daily living, status of their cognitive function, their physical health, and their economic and other resources. The Aging Care Manager 1 develops and maintains professional relationships with social service agencies and other community resources to meet the needs of clients and to ensure an appropriate continuum of client care. The Aging Care Manager 1 is supervised by the Aging Care Management Supervisor 1.
MINIMUM EDUCATION, EXPERIENCE AND TRAINING REQUIREMENTS:

A bachelor's degree which includes or is supplemented by 12 college-level credit hours in sociology, social welfare, psychology, gerontology, or other related social sciences.

Or three-and-one-half years of experience as an Aging Case Aide and an associate degree with 12 college-level credit hours in sociology, social welfare, psychology, gerontology, or other related social sciences.

Or any equivalent combination of experience and training and an associate degree including successful completion of 12 semester hours credit in sociology, social welfare, psychology, gerontology, or other related social sciences.
OTHER REQUIREMENTS:
At the time of hire, a valid driver’s license and positive driving record, a favorable criminal background check, child abuse and FBI clearances, as well as favorable references are required. The applicant must provide proof of a valid Pennsylvania driver’s license within 60 days from the date of hire. An alternative form of identification will be accepted if you are not eligible to receive a Pennsylvania driver’s license under 67 PA. Code § 83 and driving is not an essential function of the position.
