Care Manager (RN) – Children’s Program | Hybrid
2 days onsite in NYC, 3 days remote
Monday–Friday | 9:00 AM–5:00 PM
Salary: $112,000 + excellent benefits (including pension plan)
Join a mission-driven organization dedicated to supporting medically fragile children across New York. The Care Manager (RN) plays a key role in reviewing Plans of Care, conducting utilization review for HCBS services, and coordinating inpatient, homecare, medication, and DME needs. You’ll work closely with Health Home Coordinators, physicians, and community agencies to ensure each child receives timely, appropriate care.
What You’ll Do
- Review Plans of Care, CANS-NY assessments, and UM requests
- Support families and coordinate community-based services
- Partner with physicians and care teams on discharge planning and DME needs
- Conduct routine follow-up to ensure goals and services remain aligned
- Identify care gaps and escalate quality or safety concerns
- Bachelor’s degree required
- Active NY RN license
- 2+ years of acute clinical experience
- Familiarity with InterQual guidelines
- Experience supporting medically fragile pediatric populations (preferred)
