Experteer Overview As Medical Care Manager, you will coordinate HIV/AIDS patient care through an interdisciplinary team to deliver effective, timely treatment. You will develop and adjust care plans based on assessments and changing clinical needs, while ensuring adherence to standards of care. You’ll re-engage out-of-care patients, assess biomedical and non-medical barriers, and document progress in the EMR. This role involves collaboration with clinic providers and contributing to reports and community activities to support care continuity and outcomes.
