Medical Care Manager (LVN)

Experteer Overview As Medical Care Manager, you will coordinate HIV/AIDS patient care through an interdisciplinary team to deliver effective, timely treatment. You will develop and adjust care plans based on assessments and changing clinical needs, while ensuring adherence to standards of care. You’ll re-engage out-of-care patients, assess biomedical and non-medical barriers, and document progress in the EMR. This role involves collaboration with clinic providers and contributing to reports and community activities to support care continuity and outcomes. Compensation / Benefits • Coordinate care with the interdisciplinary team, patients, and families to optimize treatment • Formulate and modify care plans based on assessments and clinical changes • Identify status changes and intervene promptly • Educate patients and document in the electronic medical record • Re-engage HIV-positive patients into treatment and assess needs, including chronic conditions • Coordinate with HIV providers to address gaps in care and barriers to adherence/retention • Prepare reports for funding sources and participate in community events/meetings • Perform other duties as assigned Tasks • CA LVN license (required) • bilingual in English and Spanish (highly preferred) Key requirements •
