Summary
The ACSW/AMFT will maximize the quality of the patient’s experience and clinical outcomes while minimizing costs through an integrated, team-oriented, strengths-based healthcare approach, raising staff and trainees to their highest level of skill. This position works closely with adults and possibly youth patients and families; behavioral health, medical, and other health care staff; working with local agencies to meet patient’s multiple clinical and non-clinical health needs. The ACSW/AMFT will provide direct behavioral health treatment services for a wide spectrum of age groups and psychiatric conditions, in addition to clinical intakes, assessments, consultation, and crisis services.
Essential Functions
- Performs all job functions in a professional and courteous manner. This includes answering all general phone calls timely. Provide excellent customer service to internal and external patients by being responsive to all inquiries in a timely manner.
- Provide therapy including evidenced based practices to individuals, couples, families, and children.
- Will conduct initial behavioral health assessments, screenings and diagnose patients utilizing standardized tools and the DSM-5.
- Developed individualized treatment plans, maintained patient records, monitored progress toward treatment goals, and conducted annual reviews of active treatment plans.
- Interfaced and maintained a close working relationship with a variety of intra- and inter-agency multi-disciplinary professionals and organizations including on-staff psychiatrists and medical professionals to collaborate regarding ways to provide well-rounded treatment services that target numerous areas of need for patients.
- Engaged in individual, collateral, case consultation, targeted case management, and crisis intervention services as needed and to reduce symptomology and improve daily functioning.
- Facilitate and coordinate the appropriate services and supports to meet the needs of patients, particularly high-risk populations.
- Provide early interventions and referrals to specialty mental health services, including referral for more intensive psychological or psychiatric support services, if needed.
- Facilitate groups.
- Obtain supervision from LCSW or LMFT.
- Attend all BH and clinic meetings including supervision.
- Document in Electronic Health Records in a timely, accurate, manner.
- Maintain confidentiality regarding privileged administrative and patient information in a professional manner.
- Maintain documentation of all counseling sessions and other services in compliance with agency policies and procedures, contracts, and other regulatory requirements.
- Complies with organizational policies and procedures.

Physical Demands
This is mostly in an office setting; however, this position may require sitting for prolonged periods of time. Walking, bending, and reaching may be required as well.
Position Type and Expected Hours of Work
This is a full-time position. Days and hours of work are Monday through Friday, and hours will range from 7:30am-6:30pm.
Travel
On occasion travel will be required.
Required Education and Experience
- Master’s degree in social work, psychology, or related field (MSW, MA)
- Registered as an ACSW/AMFT with the Board of Behavioral Science and be in good standing with the board.
- CPR Certified.
- Three to five years minimum clinical experience required, and supervisory experience preferred.
- Knowledge of integrated behavioral health, mental health recovery, harm reduction, age specific growth and development, crisis and behavior management is expected.
- Working knowledge of administrative practices and procedures, HIPAA regulations, policies and standards related to behavioral health.
Preferred Education and Experience
- Non-Profit / FQHC Clinic experience
- Experience working with culturally diverse patients
- Bilingual (English/Spanish)

Pay: $32.66 - $39.33 per hour
