Experteer Overview As a Care Manager (Social Worker) at Providence Mission Hospital, you advance patient-centered care by coordinating resources and addressing psychosocial needs to help patients access appropriate services. You will collaborate with a multidisciplinary team to assess, plan, implement, and evaluate care plans, advocating for patients to optimize health outcomes. This role supports the hospital’s mission to deliver high-quality, compassionate care to vulnerable populations. You will work in a Magnet-designated, high-performing environment that values teamwork and patient advocacy.
