About Us
Cognitive Hygiene Advanced Therapy (C.H.A.T.) is a youth-focused behavioral health agency dedicated to providing high-quality clinical services, emotional support, and therapeutic structure for children and adolescents. We are seeking a PRN Assessment Specialist who can serve as the initial point of contact for new families entering our program.
Position Summary
The PRN Assessment Specialist conducts Diagnostic Assessments and completes Initial Treatment Plans for new clients entering services. This role functions as the “initial therapist” during intake, helping families understand our program, gathering detailed clinical information, and ensuring all documentation is completed accurately, thoroughly, and on time.
Assessments are completed virtually via telehealth, and both the parent/guardian and the client must be present during the assessment session.
Once the Diagnostic Assessment and Initial Treatment Plan are completed, the Assessment Specialist is responsible for coordinating with the Clinical Director and notifying the assigned in-person provider that a new client is ready for services.
Although this is a PRN/as-needed position, strong communication with leadership, providers, and the agency as a whole is essential. The Assessment Specialist is still considered part of the clinical team and is expected to stay informed about agency updates, attend staff meetings when required, and maintain familiarity with program services, expectations, and workflows.
Primary Responsibilities
- Conduct virtual Diagnostic Assessments for youth ages 6–15 via telehealth.
- Complete high-quality, clinically sound Initial Treatment Plans immediately following each assessment.
- Educate parents/guardians about C.H.A.T.’s services, expectations, attendance requirements, and program structure.
- Serve as the initial point of therapeutic contact for new families, creating a welcoming, informed intake experience.
- Communicate promptly with the Clinical Director regarding completed assessments and any concerns identified.
- Notify the assigned provider when a new client has been assessed and is ready to be added to their caseload.
- Maintain strong documentation standards consistent with Medicaid, clinical, and agency requirements.
- Participate in staff meetings, trainings, and agency updates as needed to remain informed and connected to the team.
- Demonstrate consistent professionalism, responsiveness, and reliability, even as a PRN contractor.
- Uphold confidentiality, ethical standards, and all agency policies.
Qualifications
- Master’s degree in Social Work, Counseling, Psychology, or related field.
- Independent licensure preferred (LISW, LSW, LPCC, LPC, LMFT, or equivalent).
- Experience conducting diagnostic assessments and creating treatment plans required.
- Strong clinical writing skills and ability to complete thorough, accurate documentation.
- Comfortable working with youth and families in a virtual setting.
- Excellent verbal communication, engagement skills, and professionalism.
- Ability to work independently while maintaining strong communication with the agency.
Compensation
- $100 per completed assessment, which includes:
- Diagnostic Assessment
- Initial Treatment Plan
- Notification to the assigned provider
- This is a PRN, as-needed position.
- No minimum caseload is guaranteed; assessments are assigned based on agency need.
