Join Our Compassionate Team
Are you detail-oriented, organized, and passionate about making a difference? We’re looking for a Hospice Intake Coordinator with medical records experience to help families during one of life’s most important transitions. In this role, you’ll be the first point of contact for new patients and ensure a seamless admission process.
What You’ll Do
- Coordinate hospice admissions from start to finish.
- Review and manage medical records for accuracy and compliance.
- Communicate with physicians, hospitals, and healthcare providers.
- Enter and maintain data in electronic health record (EHR) systems.
- Provide compassionate support to patients and families.
What We’re Looking For
- High school diploma or equivalent (Associate’s or Bachelor’s preferred).
- 2+ years of medical records or healthcare administration experience.
- Hospice or home health intake experience is a plus.
- Strong computer skills (EHR systems, Microsoft Office).
- Excellent communication and organizational skills.
- Ability to work in a fast-paced, empathetic environment.
- Spanish-speaking is a plus.
