Agency Background:

Individual & Family Support Network (IFSN) is a non-profit organization dedicated to providing housing and support services to individuals and families experiencing homelessness. Our mission is to empower clients to achieve stable housing and self-sufficiency through comprehensive support services.

Position Summary:

As a Housing Case Manager at IFSN, you will play a pivotal role in our rapid rehousing program, specifically focusing on individuals who are currently experiencing homelessness. You will provide intensive case management services aimed at securing stable housing and promoting independence through tailored support plans.

Key Responsibilities:

  • Conduct comprehensive assessments to determine housing needs and barriers for clients.
  • Develop and implement individualized housing stability plans in collaboration with clients, addressing goals related to housing, employment, healthcare, and other supportive services.
  • Coordinate housing placements and assist clients with lease agreements, move-in processes, and ongoing tenancy support.
  • Provide crisis intervention, advocacy, and referrals to community resources as needed.
  • Maintain accurate and up-to-date documentation of client progress and case notes in accordance with program standards.
  • Collaborate effectively with internal teams and external partners including landlords, community organizations, and government agencies.
  • Participate in regular case conferences, team meetings, and training sessions.

Qualifications:

  • Bachelor’s degree in social work, psychology, counseling, or a related field (Master’s degree preferred).
  • Previous experience in case management, preferably in housing services, homelessness prevention, or reentry programs.
  • Knowledge of issues related to homelessness, reentry, and the criminal justice system.
  • Strong communication skills and ability to build rapport with diverse populations, including individuals with histories of incarceration.
  • Ability to work independently and as part of a multidisciplinary team.

Additional Requirements:

  • Must have a valid driver’s license and access to a reliable vehicle.
  • Ability to meet IFSN’s driving policy
  • Ability to pass background checks required for working with vulnerable populations.
  • Bachelor's degree in social work, psychology, human services, or a related field (master’s degree preferred).
  • Bilingual preferred but not required.
  • Minimum of 2 years of experience working in case management, social services, or a related field, with a focus on serving individuals from the re-entry population.
  • Knowledge of housing resources, rental assistance programs, and housing laws and regulations.
  • Familiarity with housing first, trauma-informed care principles and culturally competent practice.
  • Proficiency in Microsoft Office and Google G-Suite.

Wage Range: $24-27 per hour, DOE

Schedule:

· Monday to Friday

· Full time position: 40 hours per week

· Alternative work schedules: 9/80, 4/10s, 5/8s, 4/9s with 1 half day

Job Type: Full-time

Pay: $24.00 - $27.00 per hour
