School-Based Clinical Social Worker

Location: San Francisco APA Family Support Services Office and a limited number of San Francisco

Unified School District (SFUSD) School Sites

Reports to: Clinical Director of Educationally Related Mental Health Services (ERMHS)

Position Summary:

The ASW/AMFT will provide school-linked, educationally related mental health services (ERMHS) to

children and youth with Individualized Education Programs (IEPs) in collaboration with school

districts. Under clinical supervision, the ASW/AMFT will deliver therapeutic services, case

management services, conduct psychosocial assessments, participate in IEP meetings, and

collaborate with, school staff, and other professionals to support students' emotional, behavioral, and

educational goals.

Essential Duties and Responsibilities:

● Provide individual therapy, and/or group therapy to students referred through ERMHS as directed by the Clinical Director. Or other duties assigned by the Clinical Director based on ERMHS program needs.

● Complete psychosocial assessments, assessment tools (CANS, PSC 35, MSE, etc), treatment plans, and progress notes in accordance with ERMHS program funded by the Department of Public Health (DPH) and other relevant Medi-Cal guidelines.

● Maintain an average caseload of 18-20 clients each school year or fiscal year.

● Participate in IEP meetings and collaborate with school staff, parents/guardians, and service providers to support student needs.

● Maintain communication with school personnel regarding student progress and coordination of services, while ensuring confidentiality and compliance with HIPAA/FERPA.

● Monitor and document progress toward treatment goals and adjust interventions psycho education for clients/caregivers, individual therapy, collateral meetings, case conferences as needed.

● Use a trauma-informed and culturally responsive approach in all clinical work.

● Participate in team meetings, case consultations with your supervisor/clinical director, and training as required.

● Support crisis intervention, safety planning, and risk assessments as needed.

● Provide linkages to referrals city services or other necessary community based services or resources in the San Francisco community.

● Willingness and ability to read, interpret and digest program related documents including but not limited to: CalAims, Epics Operational Manual, SFUSD, DPH materials related to the ERMHS program. Willingness to learn relevant laws and regulations related to IEP plans.

Qualifications:

▪ Minimum of one year of post graduate, professional clinical experience serving children, youth and their families.

▪ Master’s degree or higher in Human Services or related fields (e.g., Social Work, Psychology,

Marriage and Family Therapy, Mental Health Counseling, etc.).

▪ Excellent organizational and time-management skills.

▪ Proficiency in electronic health record documentation that follows Department of Public Health and Medi-Cal standards.

▪ Knowledge of child abuse and neglect and the child welfare system in California.

▪ Knowledge of healthy child development, required.

▪ Board of Behavioral Sciences registered or eligible to register, required.

▪ Experience working with immigrant children, youth, young adults from age 5-24 years old and their families, preferred.

▪ Experience in school settings (elementary school, middle school, high school) preferred but not required.

▪ Ability to provide bilingual services in Cantonese/Mandarin, or another Asian language, preferred.

▪ Travel regularly between San Francisco Unified School District (SFUSD) school sites and APAFSS office locations; demonstrate the ability to navigate San Francisco’s public transportation system effectively.

Demonstrates a willingness to grow when necessary in clinical knowledge or flexibility in ways of thinking in addressing clinical situations, programmatic implementation challenges and other work applications.

Problem Solving and Flexibility:

Be willing to be flexible while providing clinical service in a school-based setting and be willing to adapt and problem solve occasional scheduling logistics limited access to therapy rooms and demonstrate resourcefulness to utilize various treatment modalities and service delivery methods e.g. telehealth, phone sessions, clinic or office based sessions. . Demonstrates ability to accept feedback and instructions from supervisor/clinical director and ability to adjust how to deliver clinical services and meet program needs.

Commitment:

Takes proactive actions to complete tasks assigned by the clinical director, and demonstrates commitment to helping the program meet weekly, quarterly or annual programmatic goals and targets to support the success of the ERMHS program.

Job Type: Full-time

Pay: $77,000.00 - $83,000.00 per year
