BILINGUAL HOSPICE SOCIAL WORKER (MSW)

About the Role

As a Hospice Social Worker, you’ll partner with patients, families, and the interdisciplinary team to support the emotional, social, and practical needs associated with serious illness and end-of-life care.

Your responsibilities will include:

• Conducting psychosocial assessments and developing individualized care plans
• Providing emotional support, counseling, and crisis intervention
• Helping families process change, grief, and complex decisions
• Assisting with advance care planning and end-of-life discussions
• Coordinating community resources, financial support options, respite services, and additional needed support
• Advocating for patient rights, preferences, and goals
• Supporting interdisciplinary team collaboration to ensure whole-person care

Qualifications

• Master’s degree in Social Work from an accredited program.
• Minimum of 1 year in healthcare (hospice experience preferred).
• Excellent verbal, written, and organizational abilities.
• Current CPR certification required.
• Bilingual in Spanish preferred.
• Deep understanding of hospice philosophy and sensitivity to issues surrounding death and dying.
