Position: Home Health MSW

We are seeking a dedicated individual for a PRN, part-time, or per diem - based role as a home health - Master of Social Work. In this role, you will be a crucial member of the interdisciplinary home health team by providing psychosocial assessments, emotional support, and community resource coordination to help patients and their families navigate various health challenges and transitions.

Location/s: San Jose and nearby areas (Our therapists choose which locations they can cover)

Responsibilities:

• Develop individualized care plans addressing emotional, social, and environmental factors.
• Conduct comprehensive psychosocial assessments to identify patient and family needs.
• Provide short-term counseling, crisis intervention, and education to patients and caregivers.
• Collaborate with nurses, therapists, and physicians to ensure holistic patient care.
• Connect patients with community resources and advocate for services to support recovery and quality of life.
• Document all visits, interventions, and progress accurately in compliance with regulatory standards. Maintain accurate and timely documentation in the electronic health record (EHR system)
• Uphold ethical, regulatory, and compliance standards for home health social work practice in California

Job Requirements:

• Minimum 1 Year MSW Experience
• Preferably at least 1 year Home Health experience
• Must have valid professional license to practice MSW in the State of California
• Valid CPR License
• Valid Driver's License and your own reliable transportation
