Home Care Coordinator - Full Time

Job Description

The PACE Home Care Coordinator (HHA)-Permanent/ Full Time is a key member of the Interdisciplinary Team (IDT), working under the supervision of the PACE Home Care Coordination RN Manager. This role ensures seamless coordination of home-based services for our elderly participants including caregiver scheduling, delivery of medical equipment, and meal coordination.

Title: PACE Home Care Coordinator-(HHA) - Permanent/Full Time
Location: San Ysidro, CA 92173
Schedule: Monday – Friday, 8:00 AM – 5:00 PM
Compensation: $22.39 – $28.09 per hour + Benefits

Key Responsibilities

• Conduct home safety evaluations (pre-enrollment, annual, and address changes).
• Coordinate delivery of meals, incontinence supplies, DME, and caregiver services.
• Track and document services in EMR.
• Collaborate with the IDT to align services with participant goals.
• Conduct monthly visits for homebound and RCFE participants.
• Support PACE nurses with logistics and participant care needs.
• Generate reports and assist with participant satisfaction surveys.

Minimum Qualifications

• Home Health Aide (HHA) Certificate from CA Dept. of Public Health
• Certified Nursing Assistant (CNA)
• CPR & First Aid Certification (American Heart Association)
• Valid California Driver’s License
• One (1) year of experience working with frail or elderly population
• Basic computer skills (Microsoft Outlook, Excel)
