Nestled in San Francisco's Excelsior neighborhood on a nine-acre campus, the San Francisco Campus for Jewish Living (SFCJL) is a center of excellence in providing a continuum of care. This includes the Frank Residences, offering luxurious assisted living and memory care services; the Jewish Home and Rehabilitation Center, featuring long-term skilled nursing, short-term rehabilitation, and an acute psychiatric unit specialized in mental healthcare for older adults over age 55; and the Jewish Home and Senior Living Foundation.

Position Overview

The Social Worker, APU provides clinical and psychosocial support to patients experiencing psychiatric, emotional, or behavioral health challenges. This position plays a key role in patient assessment, care planning, crisis intervention, discharge planning, and coordination of services to promote stabilization, recovery, and successful community reintegration. The social worker collaborates closely with the interdisciplinary team, family, and community partners to ensure ethical, and patient-centered care in compliance with federal, state, and facility regulations.

Key Responsibilities

• Conducts psychosocial assessments, mental health evaluations, risk assessments upon admission, including social, family, housing, financial, and community factors affecting care.
• Develops and updates individualized treatment plans in collaboration with the multidisciplinary team and the patient/family.
• Facilitates group therapy, psychoeducation, and individual supportive counseling sessions focused on coping skills, relapse prevention, and emotional regulation.
• Provides crisis intervention for patients exhibiting acute psychiatric distress, suicidal ideation, or aggressive behaviors, following hospital policy and safety protocols.
• Coordinates and ensures safe discharge planning, including referrals to outpatient mental health providers, case management, residential programs, substance use treatments, and other community resources.
• Acts as liaison with families, guardians, and community agencies to promote continuity of care and address psychosocial barriers.
• Maintains accurate, timely, and compliant documentation in the electronic medical record.
• Participates in treatment team meetings, case reviews, legal hearings, and quality improvement activities to ensure optimal outcomes and regulatory compliance.
• Provides education and support to staff regarding psychosocial issues, trauma-informed care, and patient rights.
• Ensures compliance with the state and federal regulations, including HIPAA, Title 22, and CMS conditions of participation.
• Supports the facilities Quality Assurance and Performance Improvement (QAPI) initiatives by participating in audits and data collection as needed.
  
  

Requirements

• Bachelor’s degree in social work, Behavioral Science or Psychology required.
• Master’s degree in social work (MSW), Behavioral Science or Psychology preferred.
• Must have at least 2 years of experience in social services and discharge planning
  
  

Qualifications

• Strong knowledge of metal health disorders, DSM-5 diagnostic criteria, and treatment modalities.
• Proficient in psychosocial assessment, group facilitation and crisis de-escalation.
• Working knowledge of community mental health systems, social services, and other community resources programs.
• Excellent interpersonal communication, and documentation skills.
• Ability to work effectively in a multidisciplinary team and adapt to fast-paced environments.
  
  

Licensure/Certifications

• California Licensed Clinical Social Worker (LCSW) or Associate Clinical Social Worker (ASW) preferred
• Current BLS/CPR
• CPI certification or other Assaultive Behavior Management program certification within 3 months of hire
  
  

Physical Demands

• Ability to lift, push, pull a minimum of twenty-five (25) pounds.
• Ability to stand for extended periods of time.
• Must be able to assist in evacuation of residents during emergency situation.
