Mental Health Association of Nassau County (MHANC) is a not-for-profit membership organization dedicated to improving mental health in our community through advocacy, education, program development, and the delivery of direct services. Our organization is looking for a motivated individual with a calling to help adults with mental illness by becoming an Intake Care Coordinator.

The Care Coordination Intake Coordinator is responsible for all aspects of the client intake process, establishing and maintaining positive relationships with the clients and referral sources. Ensures that clients meet admission criteria and obtains proper documentation and completes admission assessments in a timely manner.

Essential Duties & Responsibilities

  • Respond to phone calls, emails and walk INS regarding program inquires and provide admission criteria.
  • Assist prospective clients in completing a SPOA application and obtain admission criteria documents from client providers.
  • Submit completed SPOA applications to the County for review.
  • Complete referrals in Cognito.
  • Coordinate with the County, Skyward Health, Northwell, on intake referrals.
  • Promptly respond to referral sources and conduct paperwork with prospective clients.
  • Document and record outreach contacts timely.
  • Complete initial assessments with consented clients.
  • Review intakes with supervisor and provide client assistance until client is assigned a care coordinator.
  • Attend community outreach events.
  • Prepare and submit reports to regulatory agencies and for internal use.
  • Submit all required documentation and reports in a timely manner.
Requirements:

Education & Experience

  • BA or BS in human service delivery required or Master’s Degree in one of the following fields: Social Work, Psychology, Education, Rehabilitation, Occupational Therapy, Counseling, Community Mental Health, Sociology, Physical or Recreational Therapy.

Other Requirements

  • Bi-lingual Spanish Required
  • Driver’s license
  • Knowledge of Medicaid, Social Security, Nassau County Resources and other Entitlements preferred.
  • Excellent Interpersonal skills required.
  • Must have the ability and willingness to regularly travel to various locations in Nassau County.
