Job Title: Medical Social Worker (PT)

Location: Fountain Hill (Operated by County of Lehigh)

We are seeking a dedicated and empathetic Part-time Social Worker to join our team. In this role, you will provide essential support to patients and their families, helping them navigate the healthcare system and access necessary resources.

Compensation

  • Base Pay: $30.06/hour
  • PT 1,000/hr a year position

Typical Examples Of Work

  • Assists in assessing needs of the resident, established written plans to meet those needs, visits/talks with residents to ascertain their problems.
  • Identifies resources, makes referrals for appropriate services, carries our social services intervention when appropriate to resolve problems.
  • Works with personnel in other facility services and offices to address and resolve residents' psychosocial needs.
  • Assists residents' families with problems that may arise.
  • Complete admission paperwork.
  • Records residents progress notes, establishes discharge plans for each resident within required time frames.

Required Knowledge, Skills & Abilities:

  • Through knowledge of social service goals, objectives & practices pertaining to the provision of services in nursing home setting.
  • Basic knowledge of organization, functions and operations of long-term nursing facilities.
  • Ability to establish and maintain effective working relationships with associates, residents, their families, representatives, and facility personnel.

Acceptable Training & Experience

  • Education equivalent to completion of a Bachelor's Degree program with equivalent years of experience in a long-term care facility.
  • One to two years of current social service experience in a health care setting.
  • Or any equivalent combination of acceptable training and experience which has provided the knowledge, skills and abilities cited above.
