What You’ll Do
As a key member of our care team, you’ll:
- Coordinate aftercare plans to ensure safe and supportive patient transitions
- Provide case management services tailored to individual needs
- Collaborate with healthcare professionals to optimize patient outcomes
- Work with patients of all ages in a fast-paced, dynamic environment
What You Bring
- Education: High school diploma required; bachelor’s degree in a human services field preferred
- Experience: Minimum of 2 years in mental health or related field
- Skills:
- Strong organizational and assertiveness skills
- Effective communication across diverse populations
- Proficiency in Electronic Health Records (EHR)
- Ability to work independently and manage tasks to completion
- Basic to intermediate computer skills
