What You’ll Do

As a key member of our care team, you’ll:

• Coordinate aftercare plans to ensure safe and supportive patient transitions
• Provide case management services tailored to individual needs
• Collaborate with healthcare professionals to optimize patient outcomes
• Work with patients of all ages in a fast-paced, dynamic environment

What You Bring

• Education: High school diploma required; bachelor’s degree in a human services field preferred
• Experience: Minimum of 2 years in mental health or related field
• Skills:
  • Strong organizational and assertiveness skills
  • Effective communication across diverse populations
  • Proficiency in Electronic Health Records (EHR)
  • Ability to work independently and manage tasks to completion
  • Basic to intermediate computer skills
