We are an established home health agency located near the Airport and Sunset Park. We are hiring a full time Spanish Speaking Bilingual Intake Care Coordinator. You will be responsible for managing and coordinating all incoming home health referrals and admissions. We are looking for someone who is able to multi-task, problem-solve, and deliver the best customer service to our patients, with dignity, respect and empathy. This role serves as the first point of contact for patients, families, hospitals, and referral sources, ensuring a timely and efficient intake process. The Intake Coordinator works closely with clinical and administrative teams to facilitate smooth transitions into home health care.
Job Duties:
- Greet and assist patients during the intake process.
- Collect and verify patient information, including medical history and insurance details.
- Schedule appointments and coordinate care plans with healthcare providers.
- Review all required forms and medical documentation to ensure they are completed accurately.
- Answer phone calls and respond to all inquiries from patients and referring healthcare providers.
- Update and maintain organized medical records in Data Soft Logic software and ensure proper filing of documents.
- Collaborate with all administrative and clinical staff via group chats to ensure a smooth intake process & all patient needs are addressed.
- Perform eligibility and benefits verification and coordination.
- Performing additional tasks, duties, and assignments as specified by your supervisor.
Education & Experience:
- High school diploma or equivalent required.
- Minimum 1 year of experience in the healthcare industry, preferably in home health or hospice intake or admissions.
- Familiarity with Medicare and Medicaid guidelines for home health eligibility.
Skills & Competencies:
- Clinical Expertise: Strong understanding of healthcare regulations and compliance standards, with knowledge and skills in maintaining schedules for healthcare providers.
- Excellent communication skills with a compassionate and professional demeanor. Ability to collaborate effectively with patients and clinical providers.
- Exceptional oral, written and communication skills. Must be organized and have high attention to detail.
- Ability to handle a high volume of calls and referrals efficiently and courteously.
- Proficiency in Microsoft Office, typing, scanning papers and electronic medical record systems.
- Ability to work independently and collaboratively in a fast-paced environment.
Working Conditions:
- Office-based with regular phone calls and computer work.
- Occasional travel to doctors offices to drop off paperwork may be required.
- Must maintain compliance with all HIPAA and confidentiality standards.
