Position Summary
We are seeking a s*easoned Hospice Community Liaison / Marketing Representative with proven hospice marketing experience and a current, active book of business*. This individual will be responsible for developing and maintaining strong referral relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, and other healthcare partners.
This is a key external-facing role that requires professionalism, integrity, and a deep understanding of hospice services and referral dynamics.
Essential Responsibilities
- Identify, develop, and maintain strong relationships with key referral sources, including physicians, discharge planners, hospital case managers, and senior living facilities.
- Present hospice services with professionalism and compassion to both healthcare professionals and the community.
- Generate new patient referrals by leveraging existing professional relationships and continuously cultivating new partnerships.
- Work closely with the clinical and administrative teams to ensure seamless coordination of patient care.
- Conduct regular field visits and outreach events to increase awareness of Aasta Hospice’s services.
- Track all marketing activity and maintain accurate records of referral contacts and outcomes.
- Participate in marketing meetings, strategy discussions, and community outreach initiatives.
Qualifications
- Required: Minimum 2 years of recent hospice marketing experience.
- Required: Must have a current, active book of business with verifiable referral relationships.
- Demonstrated success in meeting or exceeding monthly admission goals.
- Established relationships with local physicians, hospitals, SNFs, ALFs, and senior communities.
- Excellent communication, presentation, and relationship-building skills.
- Self-motivated, organized, and able to manage time effectively in the field.
- Valid California driver’s license and reliable transportation.
