Care Coordinator - LVN $41-44

We are seeking a compassionate and organized Licensed Vocational Nurse (LVN) to join our team as a Care Coordinator. This is primarily a position based at our Torrance headquarters, focused on managing the patient experience from referral through discharge. While the role may require occasional field visits, the majority of your work will take place onsite during regular business hours.

Key Responsibilities:

• Coordinate patient care from our Torrance headquarters, managing the full cycle from referral through discharge
• Conduct patient intake and admission support remotely or in-office, with occasional field admissions
• Provide triage support and collaborate with clinical staff
• Schedule patient visits and coordinate with the field team
• Order and track necessary supplies, including DME
• Maintain accurate and timely documentation
• Communicate with patients, families, and care teams primarily via phone and electronic systems
• Provide occasional in-field support if needed

Qualifications:

• Current LVN license in the state of California (required)
• Minimum 2 years of nursing experience (required)
• Valid driver’s license and reliable transportation
• Proficient in EMR systems ( HospiceMD) and general computer use
• Comfortable working onsite with occasional flexibility to enter the field as needed

Preferred Experience:

• Background in hospice, home health, skilled nursing, geriatric, or oncology settings
