Job Overview
Affinity Home Health Resources is seeking a dedicated and compassionate Intake Coordinator to join our Home Health team. This role requires strong clerical skills, excellent communication abilities, and a commitment to patient-centered care. This is an office job that requires computer tech savvy individuals.

Summary

The Intake Coordinator serves as the first point of contact for new referral sources, patients and families, ensuring efficient admission of patients into home health. This role supports patient flow, payer verification, referral relationships, and data documentation to drive quality, compliance and growth of the business.

Key Responsibilities

  • Receive and review all new patient referrals (from hospitals, SNFs, clinics, physicians, other agencies),
  • Collect and document patient demographic, clinical, insurance, and authorization information in the electronic system.
  • Verify insurance eligibility, obtain authorizations / prior‐authorizations for home health, and ensure third-party reimbursement requirements are met.
  • Coordinate initiation of care — schedule start‐of‐care visits, assign to case manager or nurse, communicate with clinical team, patient/family and referral source.
  • Maintain strong relationships with referral sources: hospitals, physicians, discharge planners, SNFs, assisted living/RCFEs, etc.; respond to questions, support smooth transitions.
  • Ensure compliance with state/federal regulations, including documentation requirements, face-to-face encounter rules (if applicable), HIPAA, home health regulatory standards.
  • Track referral/admission metrics (volumes, conversion rates, referral sources trends), provide reports to leadership.
  • Assist in process improvement: streamline intake workflows, update policies, maintain call/referral logs, ensure efficient use of resources.
  • Provide excellent customer service: respond to patient/family inquiries, relay clear information about our services, coverage, next steps.

Qualifications

  • High school diploma or GED required; Associate’s or Bachelor’s degree in Healthcare Administration, Business, Social Work, or related field preferred.
  • Previous experience (preferably 1-3 years) in a home health, intake/referral, or medical office setting. Experience with home health outcomes preferred.
  • Strong knowledge of medical terminology, insurance/authorization processes, referral sources, and EMR systems.
  • Excellent communication (verbal and written), organizational, multitasking and interpersonal skills. Ability to build relationships and coordinate across teams.
  • Proficiency with Microsoft Office (Word, Excel), and comfortable with telephones, documentation, data entry.
  • Ability to work in a fast-paced environment, prioritize tasks efficiently, and maintain confidentiality (HIPAA).
  • Preferred: Bilingual in Spanish/English (given the demographic in Los Angeles/Ventura).

Working Conditions

  • Full-time, Monday through Friday standard business hours; may include periodic after-hours or weekend coverage depending on referral volume.
  • Office-based with computer/phone work; some walking/travel to referral sites or hospitals may be required.
  • Occasional lifting of light items (files, documents) and extended periods of sitting and phone/computer use.
