Solaris HealthCare is seeking a Discharge Coordinator to join our team!:
Position Summary: The Discharge Planner is responsible for coordinating and managing the discharge process for our residents, ensuring a seamless transition from our facility to their next level of care. The Discharge Planner will work closely with healthcare providers, residents, and families to develop and implement comprehensive discharge plans that meet individual needs and comply with regulatory standards.
Key Responsibilities:
- Assessment and Planning:
- Conduct thorough assessments of residents' medical, psychological, and social needs to develop personalized discharge plans.
- Collaborate with the interdisciplinary care team, including physicians, nurses, therapists, and social workers, to ensure cohesive discharge planning.
- Coordination and Communication:
- Arrange and coordinate post-discharge services, including home health care, outpatient therapy, and community resources.
- Communicate discharge plans effectively to residents, families, and staff, ensuring clarity and understanding of next steps.
- Documentation and Compliance:
- Maintain accurate and up-to-date documentation of discharge planning activities in compliance with regulatory standards and facility policies.
- Ensure discharge plans meet all applicable state and federal regulations, including Medicare and Medicaid requirements.
- Advocate for residents' needs and preferences during the discharge planning process, ensuring their rights and dignity are maintained.
- Provide education and support to residents and families regarding post-discharge care options and resources.
- Participate in quality improvement initiatives to enhance discharge planning processes and resident outcomes.
- Stay informed about best practices and emerging trends in discharge planning and long-term care.
- Bachelors Degree in Social Work or a Human Services field included but not limited to Sociology, Special Education, Rehabilitation Counseling Psychology.
- Minimum of 2 years of experience in discharge planning, case management, or a related role in a healthcare setting, preferably in long-term care.
- Strong knowledge of Medicare, Medicaid, and other insurance regulations related to discharge planning.
- Excellent interpersonal and communication skills, with the ability to work effectively with diverse populations.
- Strong organizational skills and attention to detail.
- Compassionate and empathetic approach to resident care.
- Proficiency in electronic health records (EHR) systems and basic computer skills.
