Annually, we deliver a wide variety of services to more than 10,000 individuals of all needs, abilities, and ages - from infants to elders. Our services include waiver case management, community living services, care coordination, respite care, and community-based residential programs.We have offices throughout the seven-county metro area and offer hybrid work arrangements after completing new-hire orientation and onboarding.FLSA Status: Full-time, Hourly, Non-Exempt
FTE: 1.0 (40 hours per week)
Hiring Range: $23.67- $26.25 USD Hourly DOE
Website:Position OverviewAs a Care Coordinator, you will have the opportunity to foster trusting connections with the individuals you serve. You will play a vital role in completing assessments, developing comprehensive care plans, coordinating services, and connecting individuals with community resources to increase their independence and overall well-being. Through creative problem-solving and coordination with a diverse team of other health and social service professionals, you will be at the center of ensuring your clients have every opportunity to lead self-directed fulfilling lives.Key Responsibilities
  • Foster trusting relationships with individuals served, emphasizing empathy, effective communication, and compassion.
  • Help members of Minnesota Health Care Programs make informed choices about their health care services.
  • Conduct assessments and coordinate services to assist members in making progress toward their health goals.
  • Perform ongoing assessment, planning, referral, orientation, monitoring, and advocacy assistance to address members' medical service needs.
  • Collaborate with healthcare professionals and community resources to ensure comprehensive care for members.
  • Advocate for individuals served, assisting with waiver coordination and advocating for their rights.
  • Ensure compliance with lead agency guidelines and maintain meticulous, up-to-date documentation using multiple electronic record-keeping systems.
  • Willingness to work with diverse populations and provide culturally sensitive care.
Required Qualifications
  • A bachelor's degree (at minimum) with a major in social work, psychology, sociology, or a closely related field (such as occupational therapy, physical therapy, speech-language pathology, audiology, recreational therapy, dietician, special education, rehabilitation counseling, nursing, human services)
  • If Degree is in Social Work, license in MN must be active.
  • If Degree is in Nursing, RN status must be active. Public Health Nurse accepted.
Preferred Qualifications
  • Knowledge of MA, Medicare, and MN Healthcare programs.
  • Familiarity with community resources and providers.
  • Strong organizational skills and attention to detail.
  • Proficiency in computer basics and familiarity with the Microsoft Office suite.
  • Certified secondary language proficiency (Increased rate of pay depending on case needs).
