The Esther at Riverbend AL, located in Springfield, OR is seeking a dedicated and detail-oriented Resident Care Coordinator to provide, coordinate, and monitor high-quality care to the residents in our community.

Responsibilities of the Resident Care Coordinator

  • Exhibits leadership qualities and communicates professionally with employees, residents, family members, responsible parties, and other healthcare professionals.
  • Implement and update care and services provided to residents through individualized service plans and communication logs.
  • Coordinates, monitors, and aids with activities of daily living in a manner that supports our person-centered care philosophy.
  • Documents and reports changes and observations of resident’s needs and preferences.
  • Participate in quarterly service plan meetings.
  • Communicates with the health services team, physicians, other healthcare providers, and responsible parties as needed.
  • Address, resident, family, and responsible party concerns as needed in a timely and professional manner.
  • Recruits and coordinates training of new Care Partners and Med Techs.
  • Manages the Care Partner and Med Tech schedules ensuring there is adequate staff coverage.
  • Fill in as needed to cover open shifts.
  • Assist in daily requirements and tasks as requested.
  • Ensure residents are provided with privacy, respect, and dignity.
  • Follows standard precautions and infection control procedures.
  • Completes state required training upon hire and annually.
  • Participate in new employee orientation and monthly in-services.

Required Experience

  • Possess, at a minimum, a high school diploma or equivalent.
  • A minimum of two (2) years’ prior senior living experience preferred.
  • A minimum of one (1) year prior Caregiver and/or Med Tech experience preferred.
  • Be at least 18 years of age.
  • Basic computer skills.
  • Must be able to speak, read, and write in the English language.
