Qualifications: To perform this job successfully, an individual must be able to perform each essential core function satisfactorily. The requirements listed below are representative of the knowledge, skill, experience, and ability necessary to perform this job.

Essential Core Functions:

1. Treatment Planning

2. Discharge/aftercare planning

3. Utilization review, including submission of continued stay requests

4. Chart reviews and medical record documentation, including reports

5. Communication with patients, families, physicians, staff and external entities

6. Conduct psycho-education and focus groups with patients

7. Patient/family education

8. Testifying at court and participating in MDT meetings. Transports youth.

9. Provides additional functions as needed by program.

Job Specifications:

1. Education, Certification/Licensure, and/or Experience

a. BSW in Social Work (preferred) or BA in Counseling or Psychology. MSW/MA preferred.

b. Social Work license in State of WV

c. One year experience with children preferred.

2. Knowledge, skills, and abilities required

a. Good verbal and written communication skills.

b. Knowledge of community resources

c. Knowledge of authorization of care, child welfare and juvenile justice processes

d. Ability to work collaboratively with others

e. Ability to assess patient/family needs for treatment and discharge planning purposes

f. Customer service

g. Treatment/discharge planning.
