Position Overview
The NARA NW Registered Nurse is responsible for providing comprehensive Nursing Services to prevent disease and promote health through the application of critical thinking and the ability to exercise independent judgement to screen, assess, plan, deliver and evaluate health interventions for individuals and families. The Registered Nurse provides support and assistance to all Medical and Psychiatric Providers and in the coordination of patient care.
The Psychiatric Nurse Care Coordinator is a Registered Nurse who has clinical experience with medical and Psychiatric clients. The nurse care coordinator provides support and guidance to staff as needed. The nurse is responsible and accountable for patient coordination.

What You Will Do
- Works closely with Triage Nursing Team in monitoring incoming calls taking lead on all calls related to Psychiatry.
- Promotes clear communication amongst the care team and treating clinicians, including, Nursing, prescribing Psychiatric professional and primary care Providers.
- Takes crisis calls and consults with Psychiatric providers, and Mental Health Specialist as needed.
- Performs Chart Reviews of clients scheduled to be seen by a psychiatric provider and ensures that the clients are following through with their Plan of Care.
- Coordinates labs, EKG’s, appointments, psychiatric medication follow up, and assists in the overall management of Psychiatric patients, particularly those with chronic diseases, and those at risk of psychiatric hospitalization including administration of long acting injectables as ordered by prescriber.
- Contacts patients to report changes in medications or plan of care.
- Coordinates prescriptions as needed. Assists psychiatric providers with Prior authorizations from insurance companies, as needed.
- Provides education, and/or self- management goal setting as written in plan of care.
- Does routine and timely documentation in clients EHR of all core process work activities including, but not limited to, evidence-based chart reviews, patient calls & correspondence, and patient education activities.
- Coordinates continuity of patient care with external healthcare organizations and facilities, including the process hospital admission and discharge, working closely with the Psychiatric mental health intake specialist.
