Position Summary
The Home Care Client Care Coordinator serves as the primary liaison between elderly clients, their families, caregivers, and the home care agency. This position ensures that each client receives high-quality, personalized, and consistent care by overseeing care plans, monitoring service quality, and maintaining open communication with all stakeholders. The Client Care Coordinator plays a key role in supporting client satisfaction, compliance, and the overall success of the agency’s care delivery operations.
Key Responsibilities
Client Coordination & Relationship Management
- Conduct client intakes, assessments, and home visits to understand care needs, preferences, and goals.
- Develop and maintain individualized care plans in collaboration with clients, families, and clinical or supervisory staff.
- Serve as the primary point of contact for clients and families, addressing concerns promptly and professionally.
- Monitor client satisfaction and proactively identify and resolve service issues.
- Ensure smooth transitions of care, including start-of-service, changes in condition, or discharge planning.
Caregiver Coordination & Support
- Match caregivers to clients based on skills, personality, and care needs.
- Communicate client updates, special instructions, and expectations to caregivers.
- Collaborate with the scheduling team to ensure proper coverage and continuity of care.
- Support caregiver performance through feedback, coaching, and recognition.
- Assist with caregiver recruitment, screening, and onboarding processes
- Coordinate caregiver schedules and manage staffing assignments
- Process timesheet submissions and payroll documentation
- Maintain caregiver certification and training records
Quality Assurance & Compliance
- Conduct regular client check-ins and home visits to evaluate service quality and caregiver performance.
- Document all client interactions, care updates, and incidents accurately and in a timely manner.
- Maintain compliance with state home care regulations, agency policies, and HIPAA privacy standards.
- Participate in audits, case reviews, and quality improvement initiatives.
Administrative & Reporting Duties
- Maintain up-to-date electronic client records and care plans.
- Prepare reports on client status, service utilization, and satisfaction metrics.
- Assist in onboarding new clients, including paperwork, consents, and scheduling coordination.
- Collaborate with the billing or administrative team to ensure accurate service documentation and billing support.
Backup Caregiving Support:
- Provide direct care services to clients when caregiver call-offs occur
- Maintain flexibility to fill in for scheduled caregivers during emergencies or short-staffing situations Compliance and Quality Assurance:
- Ensure all documentation meets state and federal regulatory requirements
- Assist with agency licensing and accreditation processes
- Maintain HIPAA compliance in all client communications
- Support quality assurance initiatives and client satisfaction surveys
Qualifications
- High school diploma or equivalent required. Associate or bachelor’s degree in nursing, social work, healthcare administration, or a related field preferred.
- Minimum of 2 years of experience in home care, senior services, or healthcare coordination.
- Strong knowledge of elderly care needs, home care operations, and state regulatory requirements.
- Excellent communication, problem-solving, and interpersonal skills.
- Proficiency with Microsoft Office Suite and home care or EMR software.
- Ability to multitask and prioritize in a fast-paced, client-centered environment.

Working Conditions

Physical Requirements
- Ability to sit for extended periods while working at a computer
- Ability to perform hands-on caregiving duties including assisting with mobility, personal care, and daily living activities
- Physical capability to lift and transfer clients up to 50 pounds with proper body mechanics
- Occasional lifting of files and materials up to 25 pounds
- Clear vision and hearing for computer work and phone communications
- Manual dexterity for typing and filing tasks
- Stamina to work varying shifts when filling in as caregiver
