Job Overview
We are seeking a compassionate and experienced Medical Social Worker to join our healthcare team. The ideal candidate will provide psychosocial support, case management, and counseling services to patients across various settings, including inpatient, outpatient, hospice, and long-term care facilities. This role involves collaborating with medical professionals to enhance patient care and ensure comprehensive support for individuals facing complex health challenges. The Medical Social Worker will play a vital role in addressing emotional, social, and practical needs, promoting overall well-being and recovery.
Duties
- Conduct thorough patient assessments to identify psychosocial needs and develop individualized care plans.
- Provide clinical counseling, including behavioral therapy, mental health counseling, crisis intervention, and motivational interviewing for diverse populations such as children, seniors, and individuals with disabilities.
- Assist patients and families with coping strategies related to diagnoses such as dementia, Alzheimer's disease, PTSD, autism spectrum disorders, and other developmental disabilities.
- Facilitate group therapy sessions and participate in multidisciplinary team meetings to coordinate care.
- Manage case documentation accurately within electronic medical records (EMR) systems while adhering to HIPAA regulations and maintaining confidentiality of medical records.
- Support patients through intake processes and discharge planning across inpatient, outpatient, hospice, assisted living, nursing home, and post-acute care settings.
- Collaborate with healthcare providers on diagnostic evaluations using ICD-9/ICD-10 coding and CPT coding for billing purposes.
- Engage in clinical research activities when applicable to improve patient outcomes and advance social work practices within healthcare environments.
- Provide education on community resources, home & community care options, and support services tailored to individual needs.
- Participate in crisis management efforts during emergencies or mental health crises involving behavioral health or substance abuse issues.
Skills
- Strong knowledge of medical terminology, documentation standards, ICD coding systems (ICD-9/ICD-10), CPT coding, and EMR systems.
- Proven experience in child & family counseling, hospice care, addiction counseling, dementia care (including Alzheimer’s), PTSD care, geriatrics, pediatrics, special education, working with people with autism or developmental disabilities.
- Expertise in behavioral therapy techniques such as cognitive behavioral therapy (CBT), applied behavior analysis (ABA), crisis intervention, motivational interviewing, and psychotherapy.
- Excellent case management skills with the ability to perform patient assessments and diagnostic evaluations effectively.
- Familiarity with HIPAA compliance standards for protecting patient information and medical documentation procedures.
- Ability to work collaboratively within multidisciplinary teams in inpatient settings or outpatient clinics.
- Strong interpersonal skills for working with diverse populations across home & community care environments.
- Experience with research activities related to mental health or social work practices is a plus.
