Get in on the ground floor and be part of something new and transformative! Hiring Case Managers for a new and innovative program! We are looking for people who would be invested in assisting children who are in care transition back home to their birth parents. This position requires non-traditional work hours and the ability to provide in-home services. The ideal candidate would be comfortable working with a diverse population and have a strong sense of compassion for those overcoming major obstacles in their lives. Case Manager Responsibilities:

· Review appropriate inner agency and/or outside agency records for the completion of services

· Schedule and perform required home visits/interviews for all services provided

· Schedule outside agency visits required to review records

· Maintain an appropriate working relationship and contact with outside caseworkers, including those from other service provider agencies, CYS agencies and/or others

· Write necessary reports following visits/services

· Attend appropriate court hearings and testifying when necessary

· Provide case management services to families/children including advocacy and resource management

· Ensure deadlines are met for all above-mentioned products/services

· Ensure proper documentation of all contact/interaction with families/clients, outside agencies and colleagues

· Responsible for upholding the principles of family-oriented, trauma-informed care in all service responsibilities.

· Follow the Four R trauma informed goals: Realize how trauma can affect people, Recognize the signs of trauma, Responding in a trauma informed manner, Resisting re-traumatization.

Other Responsibilities:

· Participate in staff/case management meetings

· Alert Supervisor to any/all problems discovered in completing caseworker duties

· Complete necessary training, as outlined by the Department of Public Welfare or ACPA

· Assist with foster family recruitment, as needed

· On call after hours foster care emergency placement services as assigned

Job Requirements

· Bachelor’s Degree in related human service filed required

· Child abuse, criminal and FBI clearances for employment purposes

· Valid driver’s license, dependable car and proof of insurance and registration
