RN for Infinity Hospice Care (IHC), works primarily in hospitals where IHC holds a current contract for hospital GIP and supports the hospital inpatient palliative care service. Gains a high understanding of the IHC intake process and support transitions to both home IHC and NEVPC.
The RN promotes the hospice program, enrolls patients, and supports providers when needed to transition patients to IHC and NEVPC. The RN engages patients, families, caregivers, and updates hospital staff as needed. The RN acts as a liaison in supporting documentation, notes, and electronic information transmission to both IHC and NEVPC.
This position reports directly to the Director of Patient Care Services (DPS) of IHC as well as any assigned hospital contracted Providers and Advance Practice Nurses.

Proven success in healthcare marketing preferred
Active nursing license from an accredited program and in good standing with the
State Board of Nursing in the state of licensure
Must maintain a valid driver’s license and current automobile insurance
Responsibilities:
Can perform hospice admission for both hospital GIP and start the admission process for patients transitioning to home setting.
Ensures an excellent hospital to home hand off.
Works with social services and/or placement when most appropriate care setting is
difficult to determine.
Utilize marketing material, discuss key programs and benefits of IHC and NEVPC
with hospital patient and family
Use and promote referral screening forms and assist in presentations to case
management, facility stakeholders, and families
Assist in gathering information to expedite hospital discharge to SNFs, help resolve issues and assist in placement.
Understands IHC inpatient admission criteria.
Ensure visit feedback and patient outcomes are communicated to referral source

An active license as a Registered Nurse in good standing the state where nursing services will be performed. Graduate from an accredited school of nursing and at least one to two years experience in nursing in an acute care setting or one year of experience in home health, hospice or comparable experience is preferred. Desire to learn and a demonstration of aptitude for the hospice philosophy and comfort care required. This position reports directly to the Director of Patient Care Services.
