Hospice Social Worker Responsibilities
Hospice social workers play a vital role in supporting patients and families through end-of-life care. Their responsibilities include providing emotional support through counseling, offering practical assistance with care planning, and helping families navigate available resources. They serve as advocates for both the patient and their loved ones.
Key duties include conducting psychosocial assessments, facilitating communication between the patient, family, and healthcare team, and connecting families with essential resources—both before and after the patient’s passing.
Qualifications:
- Master’s degree in Social Work (MSW) required
 - Familiarity with Hospice MD and hospice services
 - Knowledge of IHSS (In-Home Supportive Services)
 - Willingness to conduct in-person visits
